Birth & Death Certificates

The Dearborn City Clerk’s Office processes all birth certificate requests for individuals born in the city of Dearborn. The Clerk’s Office also processes all death certificate requests for individuals who died in the city of Dearborn.

The office does not have access to and can't process requests for certificates of births or deaths that occur outside of the City.

Online Requests for Birth & Death Certificates

Birth certificates for individuals born in the city of Dearborn and death certificates for those who passed in Dearborn may be ordered online through VitalCheck. 

 

 

VitalCheck is a secure national registry for birth and death records. It is the only service that is approved by the City of Dearborn for ordering birth and death certificates online. 

Requests for Birth Certificates

Birth certificates can be ordered in person at the City Clerk's Office or by mail for a fee:

$20 for the first copy

$5 for each additional copy of the same record.  

Michigan state law requires that birth certificates can only be released to the following individuals: 

The person named on the certificate with valid photo identification.
The mother or father named on the certificate with valid photo identification.
The legal guardian with valid photo identification and signed guardianship papers.
The State of Michigan or an adoption agency with valid State/Agency photo identification.

In-Person Requests for Birth Certificates

Birth certificates may be ordered in person at the Dearborn City Clerk's Office at the Dearborn Administrative Center, 16901 Michigan Ave. 

The City Clerk's Office is open Monday through Friday, 8 a.m. - 5 p.m., excluding City holidays. 

 

 

You must provide the following to order a birth certificate in person: 

1.) Valid photo identification 

2.) Payment by cash, check or money order made payable to the City of Dearborn, or credit/debit card payment (service fee applies to all debit/credit card payments.)

Requests by Mail for Birth Certificates

Birth certificate requests may be mailed to the following address with valid payment:

If requesting a birth certificate by mail, be sure to include the following information: 

  • 1.) Birth Certificate Request Form 
  • 2.) Photocopy of valid photo identification
  • 3.) Check or money order payment made payable to the City of Dearborn only. 

Do not mail cash. The City can't process cash by mail. 

Requests for Death Certificates

Death certificates can be ordered in person at the City Clerk's Office or by mail for a fee:

       $20 for the first copy 

       $5 for each additional copy of the same record.  

Death records are readily accessible as public information. Anyone can request a legal copy of the death certificate.

To request a death certificate, you must provide the following information, along with a Death Certificate Request Form and payment: 

Name of the person who is deceased
Date of their death
